    Gathering a history allows client to be more open with therapist .It is very important for therapist too, as for better understanding of client . Some therapist make pictorial points and some make bullet points of whatever client says.
    There is structured Inquiry usually use for history taking i.e. personal history , medical history ,psychological history , social history ,risk factors.
    Most of therapist do open ended session , so it doesn’t seem like investigation to client . Use probing questions , to encourage client to explore their feelings.
    Pay attention to client body language .
    Reflect the information to show attentiveness and validation .
    Share your observation and initial thoughts with clients to ensure they feel involved during session .
    note any strong emotional reaction .
    identify patterns.
    These are few important things for history taking .
